Classes are limited in size and registration is on a first-come first-served basis. There is a $50.00 non-refundable registration fee due at the time of application. Once a class is full, you will be notified. You will be able to either rescind your application or be placed on our waiting list.
General Information – An introduction of your family to his/her teacher.
Child Enrollment and Health Information (JFS form 01234) – This is a three page form. Every item must be complete. This form must be on file before the child’s first day of attendance. No physician signature required. *If you are a returning member and the information is still correct, you will only have to sign/date the copy that is in your child’s file.
Child Release Form – Names can be added or deleted at any time, or the form may be blank. However, it must be signed. (Returning members do not need to fill out again)
Statement of Nonconviction – you are signing as the owner of the school.
Medical Statement (JFS form 01305) – Physicals are valid 13 months from the date of exam. Immunizations must be up to date. A current physical must always be on file.
3s Preschool Program – $1305.00 annually (or $145.00 monthly)
4s Preschool Program – $1305.00 annually (or $145.00 monthly)
4s Preschool Program + Enrichment day) – $1575.00 annually (or $175.00 monthly)
Tuition may be paid via check or PayPal. You can either drop a paper check off at the school or mail to PO Box 40281, Bay Village, OH 44140. You may also pay by electronic bill pay or check.
